ASSESSMENT REQUIREMENTS

Duly accomplished application form (leave Date of Application blank)
Self-Assessment Guide
3 pcs. Passport-size picture (white background, with collar)
Photocopy of Government Issued ID
Training/Industry Certificate
- for TVET Graduates
  - Certificate of Training (with Trainer's Name)
- for Industry Workers
  - Certificate of Employment (related to the qualification)
  - Photocopy of Employer's ID with 3 signatures

APPLICATION PROCEDURE

STEP 1: COMPLETE THE APPLICATION FORM

Get a copy of the Application Form at the STATC Window or download and print the form from the Downloads section below.
Accomplish the form.

STEP 2: SUBMIT REQUIREMENTS

Present the duly accomplished form and the application requirements to the Processing Officer.

STEP 3: PAYMENT OF TRAINING FEE

Pay the Assessment Fee at the Cashier.

STEP 4: PRESENT RECEIPT

Present the receipt / proof of payment to the Processing Officer.

STEP 5: WAIT FOR VERIFICATION TEXT AND SCHEDULE OF ASSESSMENT

Wait for the verification text including the necessary reminders and the schedule of training.
