APPLICATION PROCEDURE

STEP 1: COMPLETE THE REGISTRATION FORM

Get a copy of the Registration Form at the STATC Window or download and print the form here.
Accomplish the form.

STEP 2: SUBMIT REQUIREMENTS

Present the duly accomplished form and the application requirements to the Processing Officer.

STEP 3: PAYMENT OF TRAINING FEE

Pay the Training Fee at the Cashier.

STEP 4: PRESENT RECEIPT

Present the receipt / proof of payment to the Processing Officer.

STEP 5: WAIT FOR VERIFICATION TEXT AND SCHEDULE OF TRAINING

Wait for the verification text including the necessary reminders and the schedule of training.
