DAMIAN-SUSAN RAMOS SCHOLARSHIP PROGRAM REQUIREMENTS

• Barangay Certificate of Indigency with Parent/'s income (2 original copies)
• Affidavit that both parents' income is not more than  ₱30,000.00 per month (2 original copies)

• Get the Enrollment Stub from the Guidance Officer.
• Undergo the Career Assessment Interview.
• Submit the Admission Requirements.
• Input the Student Information using the Registration Kiosk.
• Go back to the Guidance Office and get a printed copy of your Student Information.
• Pay Php 150.00 to the Cashier for the Entrance Exam.
• Exam results will be discussed during the Student Interview.
• The Guidance Officer will issue the Endorsement Slip.

• Present the Endorsement Slip to the College Registrar for Assessment.
• Submit the Admission Requirements.
• Wait for the Assessment Form with Matriculation.

• Pay the Upon Enrollment Fee to the Cashier based on chosen Mode of Payment.

• Proceed to MIS Office for Bluehive Registration (Learning Management System of SMACP)

• Scan the QR Code for SMACP Social Media Pages, and like the page. 
• Watch the DSR Scholarship Orientation.

• Proceed to ID Capturing and wait for release on the same day.

• Proceed to Bookstore for purchase of Uniform and ID Lace.
